What to Expect

Location

​

We are conveniently located at 362 Park Creek Drive (The clinic is visible from Bluecutt Road.) in Columbus, and easily accessible from Highway 45 North, Highway 82, and Military Road. Click here to view a map of our location.

Parking

​

We have ample parking, including handicapped spaces, for our patients.

​

When You Arrive

​

Upon arrival, we ask that you sign-in at our front desk. You may be asked to present your ID and insurance cards, so please bring them with you.

If this is your first visit to Lowry Medical Clinic, you will be asked to complete several New Patient forms. All patients will be asked to complete a Patient Intake form. These forms can be found on this website, and it is recommended that you print them out and complete them prior to your visit. Click here to access these forms.

Payment Policy

​

Payment in full, including co-pays, deductibles, and self-pay amounts, is expected at the time of treatment. For more information on our payment policy, including a list of some of the insurance plans that we accept, click here.

Smoking

​

Lowry Medical Clinic is a non-smoking facility, and this policy is strictly enforced. If you must smoke, please step outside of the building to do so. We ask that you dispose of any cigarette butts properly. Once again, we suggest that you minimize your time away from the reception area, to avoid being absent when you are called in to the examination area.

Food and Beverage

​

We ask that you refrain from bringing any food and beverages into the clinic during your visit.

​

Supervision of Children

​

If it is necessary for you to bring children with you to the clinic, we ask that they not be left unsupervised during your visit. We also ask that you be considerate of other patients when minding your children.

​

Personal Property

​

Lowry Medical Clinic is not responsible for any personal items, such as purses, phones, etc. that are left unattended, either in the clinic or the parking lot.

​

Cancellation Policy

​

If it becomes necessary for you to cancel your appointment with us, we ask that you do so at least 24 hours prior to your visit. Cancellations within 24 hours of your scheduled appointment may result in you being charged for an office visit.

Although we will make every effort to re-schedule your visit for as soon as possible after your original appointment time, we cannot guarantee that we will be able to accommodate your requested day or time.

We will make every effort to protect your privacy rights while you are at our clinic.
